SCORE Employee Health Risk Appraisal The SCORE Employee Health Risk Appraisal takes just a few minutes to complete and your results are immediate. To gain the most benefit from this health appraisal you should have the following information available: Your Height (feet and inches) Your current weight (pounds) Your most recent cholesterol level, including HDL Your most recent, or average Blood Pressure reading (systolic / diastolic ) If you had an electrocardiogram (ECG), whether Left Ventricular Hyertrophy (LVH) was indicated. If you don't know the answer to a question, simply select "don't know" or skip the question and continue to the end of the questionnaire.
